Abstract
Background: The mortality risk of obesity in the elderly is different from that of the adults. The aim of this study is to investigate whether obesity increases the risk of mortality in the Korean elderly who took comprehensive medical checkup.

Methods: Average follow-up period is 5.0 years ranging from 24 days to 7.4 years. The significance of variables associated with death was analysed. The significance of mortality risk of the significant
variables was analysed with Cox¡¯ regression model. After adjustment for sex and weight loss, relative risk for death of the variables had significance was analysed again.

Results: Low body weight group has the highest mortality rate in all the body weight group. After adjustment for sex, weight loss, Charlson comorbidity index, age, heart rate/min, anemia, and smoking
status, relative risk for death in the low weight group compared to the normal weight group is 2.383(P value: 0.015, 95% CI: 1.186¡4.790). The mortality risk of obesity is slightly higher than that of normal weight, but not significant.

Conslusions: The mortality risk of overweight and obesity in the elderly who took medical checkup is not higher than that of normal weight. Decisions about whether or not to advise weight loss must
be individualized, with particular attention to the patient¡¯s weight history and coexisting medical conditions. A focus on physical fitness, muscular strength, and improvement of physical function may be more productive than weight loss in many cases.
